If you are interested in our services or would like to make an appointment, please click the following link to complete an Intake Questionnaire. The intake form should not take more than a few minutes and will allow us to better understand your needs.
If we are able to assist, someone from the office will reach back out to you with some follow-up questions to determine if we are a good fit for your needs and discuss scheduling.

We maintain confidentiality for all intake inquiries. However, initial phone consultations and inquiries do not establish a doctor-patient relationship.

Intakes for Reliance Psychiatry, PC and our providers are not meant for clients with medical emergencies or in acute crises.
